From the Disney's America Theme Park, Haymarket, Virginia. The park was doomed long before it was scheduled to open in 1998. Actually, the idea was scrubbed in 1994.

 

It is a blank white oval, small red border line, and the Disney's America logo on the top. Similar in size to any Cast Member nametag, the engraving would show blue letters when a name was added.
