This lanyard medallion is from Tokyo Disney Resorts cast member store. It is only available to purchase if you are a TDR cast member. Minnie is peering over the "D" that Walt used to sign Disney. The "D" is red with a silver background Minnie has white gloves on and all you can see is her face and hands and upper body. She is wearing her red polka dot dress with matching bow. The "D" is cut out so you can see through it. It is approximately 2" by 2 1/4".
